How do I activate my BioLife card?
You can also activate the Card by calling (866) 692-9282 toll free, 24 hours a day, seven days a week. Make sure you have your 16-digit Card account number and the last four digits of your Social Security Number ready when calling.
How do I recover my BioLife username?
If you have been using your Facebook account to log in, please enter the email address associated with your Facebook account below. This will be your new username. If you have been using your Google account to log in, please enter your Gmail address below. This will be your new username.
What happens if I lost my BioLife card?
Card Replacement - If you lose your card and have it replaced in the center, there is a $4.95 fee for the replacement. Point of Sale Purchase (Signature and PIN) - No fee. Over-the-counter Bank Cash Withdrawal - $2.95 fee.
