- In Forms, open a form or quiz.
- Click Add question. and enter your request for information.
- Next to the question, click the Down arrow. and select File upload.
- If prompted, review the uploading information and click Continue.

- Click ADD FORM ELEMENT and go to the WIDGETS section.
- Search for the PDF EMBEDDER widget then add it to your form.
- On the GENERAL tab, click the UPLOAD FILE button and upload your PDF file.
